I, as parent or guardian of the minor child participating in class, am aware that participation in a sport or physical exercise, such as yoga, may result in accident or injury, and hereby assume the risk connected with any participation from myself or my child in any yoga class with Ivett Almaguer or Honey Heart Kids Yoga.

I acknowledge that Ivett Almaguer or Honey Heart Kids Yoga has not and will not render any medical services including medical diagnosis of my child’s physical condition.

I hereby agree to irrevocably release and waive any claims that I and/or my child/children have now or hereafter may have against Honey Heart Kids Yoga, Ivett Almaguer, any affiliates associated with Honey Heart Kids Yoga and the park/facility in which the yoga program is held.

I specifically agree that Ivett Almaguer and Honey Heart Kids Yoga shall not be liable for any claim, demand, cause of action of any kind whatsoever for, or on account of, any kind of personal injury, property damage or loss of any kind resulting from or related to my child’s use of the facilities or participation in any exercise or activity within the Honey Heart Kids Yoga Program, and I agree to hold Ivett Almaguer and Honey Heart Kids Yoga harmless from same.

If I choose to personally participate in the Honey Heart Kids Yoga Program, I affirm all of the above stipulations for myself as well.
